Mediflow Water Pillow vs Tempur-Ergo Neck — for chronic neck pain

By the pillowed-it teamLast reviewed May 2026

Most neck-pain pillows are variations on the same theme: contoured memory foam in different densities. These two break the mold. The Mediflow uses a water-fill core that you can dial firmness on. The Tempur-Ergo Neck uses Tempur's proprietary visco-elastic foam in a sculpted cervical shape. Different mechanisms, different feels, different price tags.

The verdict in one sentence

Buy the Mediflow if you've already tried a foam contour pillow and it didn't help — the water mechanism is genuinely different. Buy the Tempur if you want the safest, most-recognized premium foam pillow and don't want to think about it.

Where Mediflow wins

Different mechanism. The water core means you can dial firmness by how much water you fill the chamber with — less water, softer pillow; more water, firmer support. The fiber layer over the chamber feels soft on your face. Mediflow is also the only pillow on this comparison that's been studied in published clinical trials for neck pain (Johns Hopkins, 1997 and follow-ups). And it costs roughly half the Tempur.

Where Tempur-Ergo wins

Tempur's proprietary visco-elastic foam is meaningfully different from the generic memory foam in cheaper contour pillows — slower rebound, more cradling, no body-heat trap once it's settled. The cervical contour is sculpted with neck-pain alignment specifically in mind. The brand is decades-trusted. There's no risk of a water leak (none, ever). And once you've slept on real Tempur, regular memory foam feels primitive.

Specs side by side

Mediflow Original Water Pillow: water-fill chamber (up to 5 lb), polyester fiber crown layer, cotton cover, mid-tier price, validated in published neck-pain studies. Tempur-Ergo Neck Pillow: TEMPUR proprietary visco-elastic foam, sculpted cervical contour shape, washable knit cover, premium price, 5-year warranty. Mediflow is half the price; Tempur is the premium foam standard.

Pick by what you've already tried

Tried a the budget tier foam contour pillow, didn't help — Mediflow (different mechanism). Tried a cheap pillow, want to step up to a premium foam contour — Tempur. Hate the slow-sink memory foam feel — Mediflow (water sits where you press it, no slow rebound). Hate the idea of water in a pillow — Tempur. Want a clinically-studied option for documented neck pain — Mediflow. Want the brand your grandmother trusts and the safest bet — Tempur. Hot sleeper — Mediflow (water carries body heat away faster than foam).

What people get wrong

The Mediflow doesn't leak in normal use — the chamber is sealed and the failure rate is genuinely tiny. Reviewers who report sloshing usually filled it under-firm with a trapped air pocket; fill it firm to the line and the sloshing goes away. The Tempur-Ergo isn't the densest Tempur pillow on the market — there are pricier ones with more support — but the Ergo is the right Tempur to buy at this price point. Don't pay extra unless you've slept on this one and felt it was too soft.
